I use my own software for formatting most of the time. Lyx generates a pdf for me for print books and I can convert and load ebooks. On my Windows laptop I’ve looked at Kindle Create and it might be a killer app, but I haven’t used it yet.

I’ve heard of D2D and looked into it. I might give it a try on an ebook and see how it goes.

For more complex formatting of print books I use Scribus. It gives me the most flexibility.
